| TERMA DAN SYARAT PENDAFTARAN KURSUS | ||||||||

| 2. | Peserta hendaklah menyediakan kad pengenalan atau pasport ASAL (bagi bukan warganegara) untuk tujuan pengesahan kehadiran. | |||||||
| 3. | Peserta hendaklah boleh membaca dan memahami Bahasa Melayu atau Bahasa Inggeris. | |||||||
| 4. | Peserta disarankan menggunakan komputer riba yang dilengkapi dengan kamera video semasa pembelajaran dalam talian. Sekiranya tiada komputer riba, peserta hendaklah menggunakan telefon pintar yang menyokong perisian latihan dalam talian. | |||||||
| 5. | Peserta hendaklah mengikuti pembelajaran dalam talian adalah secara individu, kaedah tontonan secara berkumpulan adalah tidak dibenarkan. | |||||||
| 6. | Peserta hendaklah mempunyai capaian internet yang stabil sepanjang kursus berlangsung. Peserta dianggap gagal sekiranya tidak mengikuti kursus secara lengkap yang disebabkan oleh gangguan internet. Peserta hendaklah berpakaian kemas dan berada pada persekitaran yang kondusif semasa sesi pembelajaran. (Contoh: Berada di bilik yang dilengkapi kerusi dan meja). |

| 7. | Peserta hendaklah berupaya menggunakan aplikasi Zoom, Open Learning, Online Photo Station dan lain-lain. | |||||||
| 8. | Pihak NIOSH akan membuat jemputan ke dalam ‘GROUP WHATSAPP’ melalui email peribadi atau nombor telefon bimbit peribadi sehari sebelum latihan bermula. | |||||||
| 9. | Peserta hendaklah berada didalam kelas dalam talian sepanjang masa dan aktifkan paparan video sepanjang kursus. Kehadiran akan dipantau melalui maklumat log in dan log out peserta di dalam Aplikasi Zoom. Kegagalan melakukan demikian akan menyebabkan peserta dianggap gagal melengkapkan kursus. | |||||||

i) Peserta yang lengkap 100% kehadiran dan lulus penilaian yang disahkan oleh Tenaga Pengajar layak menerima kad passport keselamatan setelah bayaran dijelaskan (kecuali kursus NTMSP dimana Penilaian Bertulis hendaklah mendapat 60% markah dan ke atas). Kad passport keselamatan boleh diambil mengikut ketetapan berikut :-

| 11. | Perlindungan insuran akan berkuatkuasa pada tarikh kursus untuk semua program passport keselamatan kecuali Program Passport keselamatan NIOSH TM (NTMSP). Bagi program NTMSP, tarikh kuatkuasa perlindungan insurans adalah pada tarikh lulus penilaian. Peserta boleh membuat semakan polisi insurans di https://verge-assist.com/ selepas 2 bulan dari tarikh kelas dijalankan. | |||||||
| 12. | Pihak pelanggan NIOSH yang membuat pembayaran atas talian sama ada melalui e-Daftar atau e-Payment di website NIOSH akan menerima: i) Notifikasi bayaran daripada bank pembayar. ii) Notifikasi NIOSH Online Payment Receipt melalui email daripada MyPay. Jika tidak terima, sila ke pautan https://mypay.com.my/receipt masukkan no. MyKad peserta ATAU no. pendaftaran syarikat ATAU no. rujukan semasa pembayaran. (Hanya menggunakan komputer atau laptop, tidak boleh melalui handphone). Pada resit MyPay yang terbaru telah dinyatakan THIS PAYMENT SLIP IS DEEMED AS OFFICIAL RECEIPTS. |

| SYARAT LULUS DAN PEMBERIAN KAD PASSPORT KESELAMATAN | |

| 1. | Peserta hanya boleh menjawab soalan penilaian sekiranya diarahkan oleh tenaga pengajar. |
| 2. | Melengkapkan 100% kesemua soalan penilaian. |
| 3. | Menghadiri sepenuhnya sesi latihan. |
| 4. | Melengkapkan sendiri penilaian. Tiada bantuan dari pihak lain. |
| 5. | Memenuhi semua kriteria penilaian yang ditetapkan khususnya memastikan jawapan yang betul bagi setiap soalan. |
| MAKLUMAT SBL KHAS | |
|---|---|
| 1. | Majikan akan dikenakan bayaran penuh sekiranya peserta dibawah Skim Latihan SBL Khas menarik diri, tidak hadir lengkap dan tidak menghadiri kursus. |
| 2. | Peserta yang menggunakan Skim Latihan SBL Khas adalah diwajibkan menghadiri 75% jam kehadiran. |
| 3. | Sijil / kad / keputusan penilaian akan di beri kepada peserta selepas pihak majikan menyerahkan borang JD14 dan borang-borang lain yang telah lengkap kepada pihak NIOSH. |
| 4. | Peserta hendaklah membawa kad pengenalan atau pasport ASAL (bagi bukan warganegara). |
| 5. | Peserta atau majikan yang telah membuat bayaran yuran kursus, tidak boleh menukar kaedah bayaran menggunakan Grant HRDC Claimable Courses (SBL-KHAS). |
| DOKUMEN PERLU DISEDIAKAN SEBELUM PENDAFTARAN KURSUS REMOTE LEARNING | |

| 1. | Membawa Kad Pengenalan/Pasport yang asal; |
| 2. | Menghantar bukti pesanan kerajaan / surat daripada jabatan melalu email jika peserta datang daripada kerajaan (siap dicetak); |
| 3. | Peserta yang menggunakan Geran SBL Khas hendaklah menghantar salinan dokumen kelulusan HRDCorp melalui email. |

| 2. | Course registration validation at the venue time begins at 8.00 am. | ||||||||
| 3. | Participants must carry an identity card or original passport (for non-citizens). | ||||||||
| 4. | Participants must be able to read and understand English language. | ||||||||
| 5. | Participants must use laptops / smartphones while attend online class. | ||||||||
| 6. | Participants must attend online learning individually; group viewing method are not allowed. | ||||||||
| 7. | Participants must have stable internet access throughout the course. Participants are considered failed if they do not attend complete course caused by internet interruptions. | ||||||||
| 8. | Participants must be able to use the Zoom, Open Learning, Online Photo Station applications and others. | ||||||||
| 9. | NIOSH will invite participant to 'WhatsApp group' via personal email or personal phone number a day before training begins. | ||||||||
| 10. | Participants must be in the online class at all times and activate the video display throughout the course. Attendance will be monitored through log in and log out information in the ZOOM application. Failure to do so will cause the participant to fail and the card will not be generated. | ||||||||
| 11. |
i) Participants are eligible to receive the safety passport card if they complete 100% attendance, pass written assessment verified by the Trainer and after the payment is cleared. (except for the NTMSP course where the Written Assessment must get 60% marks and above). The safety passport card can be taken as per below :-

| 12. | Insurance coverage will be effective on the course date for all safety passport programs except the NIOSH TM Safety Passport Program (NTMSP). For the NTMSP program, the effective date of insurance coverage is on passing date of assessment. Participants can review the insurance policy at https://verge-assist.com/ after 2 months from the date of the class. | ||||||||
| 13. | Participants who make payment online either through e-Register or e-Payment on the NIOSH website will receive: i) Payment notification from the paying bank. ii) NIOSH Online Payment Receipt notification via email from MyPay. If not accepted, please go to the link https://mypay.com.my/receipt , key in the Participant's MyKad number OR company registration number OR reference number during payment. (Only use computer or laptop, not via mobile phone). On the MyPay receipt it has been stated THIS PAYMENT SLIP IS DEEMED AS OFFICIAL RECEIPTS. |

| CONDITIONS FOR PASSING AND ISSUE OF SAFETY PASSPORT CARD | |

| 1. | Participants can only answer assessment questions if instructed by the trainer. |
| 2. | Complete 100% of all assessment questions. |
| 3. | Fully attend training sessions. |
| 4. | Complete your own assessment. No help from other parties. |
| 5. | Meets all the prescribed evaluation criteria in particular ensuring the correct answer to each question. |
| SBL SPECIAL TRAINING SCHEME INFORMATION | |

| 1. | The employer/sponsor of the participant will be charged in full if the participant under the SBL Special Training Scheme withdraws, does not attend completely and does not attend the course. |
| 2. | Participants who use the SBL Special Training Scheme are required to attend 75% of the attendance hours. |
| 3. | Certificates / cards / evaluation results will be given to participants after the employer/sponsor submits the JD14 form and other completed forms to NIOSH. |
| 4. | Participants or employers that already paid the course fee cannot change the payment method to Grant HRDC Claimable Courses (SBL-KHAS). |
| DOCUMENTS MUST BE PROVIDED BEFORE REGISTRATION FOR REMOTE LEARNING COURSE | |

| 2. | Send proof of government order / letter from the department via email if participants come from the government (ready to print). |
